THE FOLLOWING CRITERIA WILL BE STRICTLY ADHERED TO IN ORDER TO PROVIDE FLU SHOTS TO PEOPLE AT GREATEST
RISK
- Individuals with one or more chronic medical conditions requiring frequent or ongoing medical management. This includes those individuals with a chronic disorder of the lungs or cardiac system, and those with chronic metabolic diseases, renal dysfunction, hemoglobinopathies or immunosuppression. You must show proof from your health care provider specifying your high-risk condition.
- No allergies to eggs or egg products.
- No history of Guillain-Barre Syndrome (GBS).
- There is no fee for the vaccine
- Children under 18 MUST be accompanied by parents/guardians.
- For Children under 10 receiving second dose of H1N1 vaccine you must provide documentation of the first dose to ensure proper administration.
