THE FOLLOWING CRITERIA WILL BE STRICTLY ADHERED TO IN ORDER TO PROVIDE FLU SHOTS TO PEOPLE AT GREATEST RISK
- Individuals with one or more chronic medical conditions requiring frequent or ongoing medical management. This includes those individuals with a chronic disorder of the lungs or cardiac system, and those with chronic metabolic diseases, renal dysfunction, hemoglobinopathies or immunosuppression. You must show proof from your health care provider specifying your high-risk condition.
- No allergies to eggs or egg products.
- No history of Guillain-Barre Syndrome (GBS).
- No child under 18 years old (please see your pediatrician).
- No pregnant women (please see your obstetrician).
- $15.00 Fee for no Insurance
